Helpful Documents

In order for us to prepare your documents for submission to the court on your behalf, please download and print the following  forms. If you cannot open a document, please call 866-204-7557 and a representative can email or fax you any document you may require. These files are in Adobe Acrobat format. You can download Adobe Acrobat for free if you don’t have it from Adobe’s Website.

1. Trial By Declaration: Print 3 copies for each ticket and sign, do not fill out these forms, sign only

2. Document Service Contract: Print 1 copy for each ticket, sign and print your name and date

3. Court Delivery Authorization: Print 1 copy for each ticket and select only ONE option, sign and print your name and date

4. Credit Card Authorization: Print 1 copy for each ticket, sign, print your name and date

5. Pay by check form: Print 1 copy for each ticket, mark ticket package(s), print your name, phone number, check # and amount

Other documents that may be helpful include:

1. TR220 or Request for New Trial
2. DL207 or Driver License Record Correction Request
   (often used to request an erroneous point be removed from your driving record)
3. Request for Refund
